Transition Alliance Programs (TAP) were created in partnership between the Division of Vocational Rehabilitation Services and Cedar Falls and Waterloo Community Schools. Youth participating in TAP are provided opportunities to develop or enhance skills in academic, vocational and independent living competencies. TAPs address the individualized needs of youth to continue as lifelong learners and become successful, productive and contributing citizens.

Effective collaboration between TAP, schools and community resources helps youth establish relationships and build effective teams. This allows TAP participants to become aware of opportunities within their community and develop additional skills that benefit them for life.

This Community Resource Map (CRM) website is sponsored by the Transition Alliance Programs of Cedar Falls High School and Waterloo Community Schools. TAP and representatives from local agencies compiled available resources/services into this user-friendly, comprehensive guide. We invite you to explore the following links on this website to research and develop an individualized transition map for your future.

Project REACH
Project REACH was developed by the Self-Advocacy & Self-Determination Subcommitee of the Transition Task Force. The goal of this project is to coordinate and recruit speakers that will visit area schools to provide discussions on transitioning to the "real world" after high school.
