WHAT IS PERMANENT PLANNING, INC.

Permanent Planning, Inc. (PPI) is a non-profit, charitable corporation organized under the laws of Iowa in 1973. Its purpose is to assist in the planning and implementation of plans for lifetime care of persons with disabilities, including, when necessary, a corporate guardianship program. Property ownership and management fund PPI’s guardianship program. The first group home was built using donated funds and then that group home was used as collateral to build a second home and the process continued until 10 group homes were built and a 12-plex and a residential home were purchased. Those properties are leased to a non-profit organization serving people with disabilities. The income generated from these properties provides revenue for support staff who provide guardianship services.

WHAT DOES THIS MEAN?

This means families can receive counseling and guidance in working out the best possible plan of care for their family member. It means that when families are no longer able to care for their family member, they will have someone to look after their loved one’s well-being. Finally, it assures that their loved one will be assisted by professional staff members who know of the services available for persons with specialized needs and who will see that each individual receives support, dignity and care throughout his or her lifetime.

WHAT IS GUARDIANSHIP?

Essentially, guardianship is a court-authorized relationship where the court appoints a person or corporation to assume responsibility over specific areas of a person’s life. Permanent Planning, Inc., could be the corporation appointed as guardian for your family member. Our goal as guardian is to follow a course of action which is in the person’s best interest, including maximizing individual growth, independence, productivity, community integration and autonomy.

HOW IS THIS ACCOMPLISHED?

Permanent Planning, Inc., will provide a professional staff person who, acting in conjunction with other professionals in the area, will maintain an ongoing evaluation of the person. Initially, by working with the family and other professionals, an individual plan for the future will be drawn up based on medical, mental health, vocational, educational and housing needs, as well as the available financial resources. When appropriate, PPI will accept guardianship of the individual, including any and all responsibilities assigned by the courts, so it will be in a position to provide a permanent and knowledgeable advocate who will see that maintenance and care is adequately provided.

You and your family member will be provided with emergency consultation and assistance during the lifetime of the person enrolled. Permanent Planning, Inc., will, during a crisis created by the disability or death of a family member, provide immediate service to the individual and his or her remaining family by assisting in planning for short-term and long-range needs, as well as in adjusting to the loss of a family member and its implications.

Through its staff members, PPI will provide a trained advocate to maintain close contact with your family member. Staff members will work with the individual and respond to his or her interests. Staff members will remember birthdays and holidays, as well as facilitate problem solving in programming or other areas. As much as possible, staff members will provide some of the personal care which family would normally provide to ensure a person’s emotional and psychological health and happiness.

Although Permanent Planning, Inc., will not provide financial assistance to the individual, PPI staff will seek assistance from current or future governmental agencies held responsible for providing such assistance to individuals, in addition to what is provided by the family through trusts, wills, social security and other resources when this assistance is necessary, as well as appropriate.

Permanent Planning, Inc., will, upon request, provide assistance to guardians, trust organizations, conservators, courts and others who have custodial responsibilities for individuals with disabilities to help plan their future and ongoing care.

WHO IS ELIGIBLE?

The services of PPI will be offered only to those it believes can benefit from its services and who meets the eligibility requirements. The eligibility requirements are as follows:

  1. The individual must have a primary diagnosis of mental retardation at the time of application.
     
  2. The individual’s county of legal settlement must be Black Hawk County at the time of application.
     
  3. The individual must reside in Black Hawk County or the surrounding counties at the time of application.

Any adult who is responsible for the care of an individual that meets the eligibility requirements may make an appointment with a PPI staff member to learn more about the services.

ENROLLMENT IS AFFORDABLE

The fee to enroll an individual in Permanent Planning, Inc., is $1,500.00, which is payable over five years.

Before a person is accepted into the program, a thorough evaluation is required of all information available concerning the individual. This will include visits with parents and a background study to obtain a complete picture of the individual’s needs and abilities. After the evaluation, both parents or guardians and the Board of Directors of Permanent Planning, Inc., have the option to enroll or not to enroll the individual.
