Gold Star Award-Nomination Form

Nominations may be submitted using the Gold Star Nomination form below. Nominations are due by midnight, March 5, 2003. Late entries will not be considered. Nominees will be notified and invited to complete the application process. Winners of The Gold Star Awards will be notified in May, 2003. The Gold Star Awards and cash prizes are not transferable. Decisions of the Gold Star Awards Committee are final. Please direct all questions to Linda Klinger, Executive Director of the McElroy Trust at (319) 287-9102.   Nominations can also be deposited in a Gold Star box at Black Hawk County schools or mailed and postmarked by midnight, March 5, 2003.
