GRANT APPLICATION GUIDELINES

there is not an "application form"
please submit your grant request in outline form, according to the following format:

I. General Information.

A. Name of Project.
B. Name of Organization.
C. Address.
D. Names and qualifications of persons who will administer the grant.
E. Contact person, title, telephone number, fax number and e-mail address.
F. Attach a copy of Determination Letter (verification of 501(c)(3) tax-exempt status) and a copy of Article of Incorporation.

II. Background of the Organization.

A. Statement of purpose.
B. Description of the organization and its activities.
C. Organization's utilization of volunteers, including number of volunteers and in what capacities they serve.

III. Project Description.

A. Community need and benefits to be derived by the community.
B. Long-terms goals of the project.
C. Specific, short-term, measurable objectives.
D. Specific activities planned.
E. Number of young people to be served and from what age group.
F. Timetable for the project.
G. Project evaluation plan, including specific outcomes expected.

IV. Financial Information.

A. Attach a copy of the organization's most recent audited financial statement.
B. Attach a copy of the project budget including for example:
  1. Payroll (hourly rate).
  2. Payroll taxes and fringes.
  3. Materials and supplies.
  4. Taxes.
  5. Rent.
  6. Transportation.
  7. Utilities.
  8. Miscellaneous.
    Line items in project budget may vary depending on type of project.
C. Amount requested from McElroy Trust and desired timing of grant payments.
D. Amount requested from all other funding sources.
E. Plans for on-going funding.
